Job Description ABCOR HOME HEALTH IS HIRING FULL-TIME RN'S TO JOIN OUR TEAM Areas of Service: Hoffman Estates, Algonquin, Arlington Heights, Barrington FULL-TIME Position with Benefits and Mileage Reimbursement PRIOR HOME HEALTH EXPERIENCE PREFERRED APPLY TODAY Position Summary: To provide nursing care in the patients--- home on an as-needed basis in accordance with the patient's plan of care, in collaboration with the primary physician. One of Abcor---s missions is to assist patients with their physicians to manage their medical conditions to remain at home. As part of the team, you will be educating patients and family members to manage their medical care and remain in their homes. Abcor provides care for high-tech patients post-hospitalization including infusion, advanced wound care, and post-cancer treatment. Responsibilities: Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for the home care patient Submit clinical documentation as directed by the Director of Nursing in a timely manner Discuss with the appropriate supervisor the need for the involvement of other members of the health team Work closely with admission clerk to coordinate patient admission Collaborate with physicians, other agency staff, and contract services to coordinate and implement the treatment plan. Re-evaluate the plan on an ongoing basis and report condition changes Qualifications: Must be a registered nurse with the Illinois Department of Professional Regulation Prior Home Health experience IS PREFERRED Must have a driver---s license and be willing and able to drive to patient---s residences.
