Type: Staff Work Schedule: 100% FTE, days. You may be eligible for
up to a $3000 sign-on bonus. You will work at 2275 Deming Way. This
is a hybrid position. You will work in person a minimum of 3 days
per week. Pay: - The pay range listed reflects both titles and is
based on a 1.0 FTE. - This position may be eligible for a $3000

independent healthcare provider, you'll play an integral role in
our care teams by providing high-quality, cost-effective care to
patients while working collaboratively within a multidisciplinary
health team. We are seeking a Behavioral Health Therapist to: -
Evaluate patients presenting with a wide range of mental health
concerns and diagnoses. - Collaborate with patients on
individualized treatment plans with specific goals and outcomes. -
Coordinate patient care with primary care, other specialty
providers, and families via written and verbal communication. -
Provide psychotherapy, utilizing multiple modalities and methods
consistent with evidence-based practice, including specific and
focused treatment protocols, when appropriate. - Utilize progress
monitoring and outcome measurement tools to assess benefits of
psychotherapy and change the treatment plan, if necessary. At UW

Therapist: Education: - Minimum – Master's Degree in Social Work,
Counseling, Psychology, Marriage and Family Therapy, or other
behavioral health related field Work Experience: - Minimum – 2
years of experience providing evidence-based treatment for a
variety of behavioral health diagnoses for children, adolescents,
or adults - Preferred – 5 years of experience providing
evidence-based treatment for a variety of behavioral health
diagnoses for children, adolescents, or adults Licenses and
Certifications: - Licensed Clinical Social Worker Upon Hire
Required or - Licensed Professional Counselor Upon Hire Required or
- Licensed Marriage and Family Therapist Upon Hire Required and -
Basic Life Support/CPR within 6 months Required If Clinical
Psychologist: Education: - Minimum – Doctor of Philosophy (Ph.D.)
or Psychology (Psy.D.) in Counseling or Clinical Psychology or
related field. Work Experience: - Minimum – One (1) year of
postdoctoral supervised clinical experience Preferred Licenses and
Certifications: Minimum – - Licensure or eligible for licensure as
a Psychologist in Wisconsin - Basic Life Support/CPR within 6
