Job Description:
SSM Health, a leading not-for-profit Catholic health system, is
seeking a dedicated full-time Registered Nurse for our Home Health
division. This role entails providing top-notch direct patient care
in home settings. Our Registered Nurses play a critical role in
patient outcomes and embody our commitment to compassionate service
across the Midwest. You will implement physician-prescribed care
plans, manage patient medications, and ensure the highest standards
of patient safety and health. Assess patients' condition during
every visit and chart your observations. Perform evaluation tasks,
including vital signs and medication review. Administer and manage
medication as prescribed by the patient's physician. Educate
patients and their families on proper home health care strategies
and procedures. Coordinate with physicians and other healthcare
professionals for creating and evaluating customized care plans.
Ensure quality assurance and patient satisfaction through
compassionate care. Strong interpersonal and communication skills
Proficient in time management Able to make decisions in emergency
situations High level of proficiency in handling medical equipment
Medical Insurance Dental Insurance Vision Insurance Life Insurance
401k Current Registered Nurse (RN) license Basic Life Support (BLS)
and Advanced Cardiac Life Support (ACLS) certification Valid
driver's license and reliable transportation as this role requires
travel to patient homes
